Le Roy and the Social Media Tic Outbreaks

Mass social media induced illness; MSMI; TikTok tics; functional tic-like behaviours

2011 – present · Le Roy, New York; then Germany, Britain, Australia, Canada and beyond

XVI. CONTAGION — SYMPTOMS THAT SPREAD WITHOUT A PATHOGEN

None.

Who led it

No leader. The clinical description of the newer phenomenon is chiefly KIRSTEN MÜLLER-VAHL'S (L289) and colleagues, in BRAIN in 2022.

What it worshipped and taught

Not doctrine. FUNCTIONAL NEUROLOGICAL SYMPTOMS — tics, verbal outbursts, seizure-like episodes, speech difficulty — that are involuntary and genuinely disabling but do not arise from the disease they resemble. THE CRITICAL POINT, AND THE ONE MOST OFTEN GOT WRONG: THIS IS NOT FAKING. Functional symptoms are not under the patient's control.

Who was in it

The roughly 20 affected at Le Roy, mostly teenage girls; ERIN BROCKOVICH, who investigated an environmental cause and did not find one; Kirsten Müller-Vahl and colleagues; the social media accounts that became index cases.

How it ended, or where it stands

LE ROY RESOLVED WITHIN A SCHOOL YEAR. The social-media phenomenon has no recorded end, and the conditions that produced it have not gone away.

The dead

Of note

THIS IS THE ENTRY WHERE THE MECHANISM STOPS NEEDING A ROOM.

Every earlier outbreak in this era required physical proximity. A convent, a village, a factory floor, a mission boarding school, a high school. That is why closing the school was the obvious move in 1962 (167), and why it was the wrong one. MASS SOCIAL MEDIA INDUCED ILLNESS NEEDS ONLY AN ACCOUNT. The index case is a person the affected have never met, in another country, and the affected are not connected to each other at all.

IT ALSO INVERTS THE OLD DEMOGRAPHY OF EXPOSURE. Previously the people at risk were the people you sat next to. Now they are the people who watch what you watch, which is a group selected for similarity of interest rather than of geography — A FAR MORE EFFICIENT TRANSMISSION MEDIUM THAN A CLASSROOM. Every structural feature that made a closed community dangerous in the rest of this dossier is reproduced here without walls.

THE ETHICAL POINT IS NOT OPTIONAL AND THIS FILE WILL NOT SOFTEN IT. Calling this psychogenic is not calling it fake, and it is not an accusation. Functional neurological disorder is real, involuntary, disabling, and treatable; the people at Le Roy were ill. THE HARM DONE BY THE OPPOSITE ASSUMPTION IS DOCUMENTED IN THIS DOSSIER — see the whole of era XV, where patients who could not be believed were shocked, operated on and locked up.

AND ERIN BROCKOVICH SHOULD NOT BE MOCKED FOR LOOKING. A town with sick children and a real historical chemical spill nearby had every reason to want it tested. It was tested. It was not that.

Its timeline

  1. OCTOBER 2011 At LE ROY JUNIOR-SENIOR HIGH SCHOOL in western New York, students begin developing tics, verbal outbursts and seizure-like episodes. TWENTY PEOPLE ARE AFFECTED BY JUNE 2012: NINETEEN STUDENTS, EIGHTEEN OF THEM GIRLS, AND ONE 37-YEAR-OLD WOMAN. Treating neurologists diagnose conversion disorder.
  2. LATE JANUARY 2012 ERIN BROCKOVICH IS CALLED IN and investigates contamination from a 1970 RAILWAY DERAILMENT near the town as a possible cause. The families wanted a physical explanation, which is an entirely reasonable thing to want.
  3. 2012 STATE AND SCHOOL ENVIRONMENTAL TESTING FINDS NO ENVIRONMENTAL OR INFECTIOUS AGENT capable of causing the symptoms.
  4. OCTOBER 2011–JUNE 2012 The outbreak runs its course. IT IS DESCRIBED AS THE FIRST SUCH OUTBREAK IN WHICH REPORTS SPREAD SUBSTANTIALLY THROUGH SOCIAL NETWORKS RATHER THAN ONLY FACE TO FACE — the affected watched one another on video.
  5. 2019 onward A GLOBAL RISE IN SUDDEN-ONSET, SEVERE, TOURETTE-LIKE BEHAVIOUR IN YOUNG PEOPLE, tracking the popularity of tic content online. The presentations resemble each other far more than they resemble Tourette syndrome, and IN GERMANY clinicians trace an outbreak to a single “virtual” index case: one of the country’s most popular YouTube creators.
  6. 2022 MÜLLER-VAHL AND COLLEAGUES PUBLISH IN BRAIN, arguing this is not Tourette syndrome but a new form of mass sociogenic illness, and proposing the term MASS SOCIAL MEDIA INDUCED ILLNESS.
  7. SEPTEMBER 2022 The same group describes 32 patients in detail. Nearly all have identifiable psychological stressors, and 94 per cent have other psychiatric symptoms such as anxiety or depression.

Its people

Kirsten Müller-Vahl

—

Neuropsychiatrist; described mass social media induced illness · — · active 2019–present

A German neuropsychiatrist specialising in Tourette syndrome who, with colleagues, described the wave of sudden-onset Tourette-like presentations appearing from 2019 as a NEW FORM OF MASS SOCIOGENIC ILLNESS RATHER THAN AS TOURETTE SYNDROME, and proposed the term MASS SOCIAL MEDIA INDUCED ILLNESS. Their 2022 paper in BRAIN set out the case: the presentations resembled each other and specific online content far more than they resembled Tourette, and in several countries patients reproduced the particular movements and phrases of identifiable creators. Subsequent prospective work compared these patients with Tourette patients and found them clinically distinguishable.

Before the law. None.

Where things stand. No death record was found at the time of writing, and this file makes no claim about her current post. HER DISTINCTION MATTERS CLINICALLY AND NOT ONLY ACADEMICALLY: these patients are genuinely ill and are helped by different treatment than Tourette patients, which is why getting the label right is not a semantic exercise.

  1. 2019 A global rise in sudden-onset functional tic-like behaviour begins, tracking the popularity of tic content online.
  2. 2022 Publishes in BRAIN, proposing MASS SOCIAL MEDIA INDUCED ILLNESS.
  3. 2022–present Prospective cohort work distinguishes the presentation from Tourette syndrome.
