The Somatic Treatments

Fever therapy; insulin coma; Metrazol convulsive therapy; unmodified ECT

1917 – c.1960 · Austria, Poland, Hungary, Italy; then worldwide

XV. THE ASYLUM — WHEN MEDICINE DECIDED WHOSE MIND WAS WRONG

None.

Who led it

JULIUS WAGNER-JAUREGG (malaria therapy, Nobel Prize 1927); MANFRED SAKEL (insulin coma, 1933); LADISLAS MEDUNA (Metrazol, 1934); UGO CERLETTI and LUCIO BINI (electroconvulsive therapy, Rome, 1938).

What it worshipped and taught

A shared premise with almost no evidence behind it: that a violent physical shock to the body — a fever, a coma, a seizure — could reset a disordered mind. THE TREATMENTS PRECEDED ANY UNDERSTANDING OF WHY THEY MIGHT WORK, and were adopted at speed because the alternative was custody with nothing to offer.

Who was in it

Wagner-Jauregg; Sakel; Meduna; Cerletti and Bini; the patients, who were rarely asked.

How it ended, or where it stands

ALL ABANDONED EXCEPT ECT, which survived by being changed almost beyond recognition. Malaria therapy was superseded by penicillin, having genuinely saved lives first.

The dead

Insulin coma and Metrazol both killed patients; no reliable total exists, and this file will not invent one.

Of note

THIS FILE EXISTS TO STOP THE ERA BEING READ AS A SIMPLE HORROR STORY.

Two things are true at once. Wagner-Jauregg deliberately gave people malaria, and it was the correct treatment for a disease that otherwise killed them. Insulin coma was administered for twenty years, and stopped being defensible once a controlled trial in 1957 showed the insulin itself did nothing. THE DIFFERENCE BETWEEN THE TWO IS NOT ETHICS OR INTENTION — both were bold, both were dangerous, both were given to people who could not meaningfully refuse. THE DIFFERENCE IS THAT ONE WAS TESTED.

AND ECT MUST NOT BE MISREPRESENTED HERE. Unmodified ECT, given without anaesthetic to a conscious patient, was a genuine cruelty and is part of this era's record. Modern ECT is a different procedure with anaesthesia and muscle relaxants, it is regulated and consented, and IT IS ONE OF THE MOST EFFECTIVE TREATMENTS PSYCHIATRY HAS for severe depression and catatonia. Conflating the two is a factual error, and it is one this dossier declines to make.

Its timeline

  1. 1917 WAGNER-JAUREGG DELIBERATELY INFECTS PATIENTS WITH MALARIA to treat general paresis, the terminal neurological stage of syphilis. IT ACTUALLY WORKED — the induced fever attacked the organism — and it was, before penicillin, the only effective treatment for an otherwise fatal condition. IT WAS ALSO DANGEROUS: about 15 per cent of patients died of the treatment. He received the NOBEL PRIZE IN 1927.
  2. 1927 onward MANFRED SAKEL introduces INSULIN COMA THERAPY: large insulin doses drive the patient into hypoglycaemic coma, repeated daily over weeks. Estimates of the death rate run from about 1 to 4.9 per cent. It was in wide use for two decades. In 1953 HAROLD BOURNE publishes THE INSULIN MYTH in The Lancet, and in 1957 a controlled trial finds that barbiturate unconsciousness does just as well: INSULIN WAS NOT THE ACTIVE INGREDIENT.
  3. 1934 LADISLAS MEDUNA induces seizures chemically, first with camphor and then with METRAZOL (cardiazol), on a theory that epilepsy and schizophrenia were antagonistic — WHICH WAS WRONG. Patients reported overwhelming dread in the seconds before the seizure, and the convulsions were violent enough to cause SPINAL FRACTURES.
  4. 1938 UGO CERLETTI and LUCIO BINI administer the first ELECTROCONVULSIVE THERAPY to a human patient in Rome. Electricity replaces Metrazol because it is faster and the dread is removed by the immediate loss of consciousness.
  5. 1940s–50s UNMODIFIED ECT — no anaesthetic, no muscle relaxant — is given at enormous scale, causing fractures and terror, and is used in some institutions for compliance rather than for illness.
  6. 1950s onward Antipsychotic drugs arrive and the shock therapies collapse, except one. ECT, GIVEN UNDER ANAESTHETIC WITH MUSCLE RELAXANTS, REMAINS AN ESTABLISHED AND EFFECTIVE TREATMENT for severe depression and catatonia, and is not the procedure described above.
